New Release: Diabetes Mellitus Toolkit

The Diabetes Mellitus Toolkit is designed to assist registered dietitians in implementing ADA's Diabetes Type 1 and 2 Evidence-Based Nutrition Practice Guideline for Adults.

This new resource includes:

  • A sample initial and follow-up progress note for documentation that utilizes the ADA Nutrition Care Process and Standardized Language.
  • An interactive progress note option for documenting electronically into a PDF file.
  • A case study highlighting the care of a generalist, specialist and advanced registered dietitian.
  • An outline of the encounter process for patients/clients with diabetes for initial and follow-up encounters.
  • Sample forms for food recall, encounter contract and client agreement for care.
  • Forms to monitor outcomes for 50 patients/clients which contain automatic calculations and graphs illustrating the progress of anthropometric, behavioral and biochemical outcomes.

The toolkit was written by ADA members Rosalyn Haase, MPH, RD, CD, CDE, BCADM; William I. Swan, RD, LDN; and Naomi Wedel, MS, RD, CDE, BC-ADM.
