A limited number of sleeping rooms have been reserved at the Sheraton Austin Hotel at a rate of $189 single/double for the evenings of June 4, 5 and 6. To make your reservation, please call the Sheraton Austin Hotel at 512/478-1111 by May 4, 2008 and mention that you are attending the ADA weight management program. You will need to guarantee your reservation with a major credit card. Payment of all room, tax and incidentals will be the responsibility of each individual.
*Registration includes the home study module and 2-1/2 day on-site workshop, including beverage breaks and post-test (meals are not included). If you have already completed the home study module, the price of the module may be deducted from the program price. Because the online registration process cannot accommodate this reduced price, please contact the CDR directly to register for the program at 800/877-1600, ext. 4783.
