State Carrier Information

To oversee administration of Medicare Part B services at the state level, the Centers for Medicare & Medicaid Services (CMS) establishes contracts with local insurance companies. These insurance companies, called carriers, perform provider enrollment, claims processing, audits and other Medicare activities. RDs interested in becoming Medicare providers for medical nutrition therapy (MNT) should contact their local carrier for a Medicare provider enrollment packet. Additionally, RDs can contact the carrier for questions on enrollment, claims processing and other carrier specific activities. The carriers’ names, phone numbers and e-mails are listed below. Depending on your question, you may be directed to another department within the carrier, such as the Electronic Claims Department, or Customer Service for specific billing questions and problems.

If you are experiencing difficulty obtaining information, or resolving a question with the carrier, RD Medicare providers can contact the Medicare regional office for assistance. The regional offices have a direct oversight role and serve as the point of contact and outreach with state and local professional organizations. They also monitor the performance of the carriers and fiscal intermediaries. Please contact them only if you fail to obtain satisfactory assistance from the carrier or fiscal intermediary.
