ADA and AADE Medicare diabetes services order form and backgrounder information for physicians

The American Dietetic Association (ADA) and the American Association of Diabetes Educators (AADE) have completed revisions to the Diabetes Services Order Form.  While the original form was released in May, after further discussions with the Centers for Medicare & Medicaid Services, we realized some additional descriptors were needed within the diabetes self-management training (DSMT) section on the form.  That area has been expanded to include check-off boxes describing the type of DSMT training services, the hours of DSMT training requested, and the DSMT content. Another new feature of the form includes designation of the sections on the referral form that must be completed for compliance with the Medicare referral for DSMT or medical nutrition therapy (MNT).  Other sections of the form are not changed from the earlier released document.

An equally, if not more important element to increasing access to Medicare diabetes services is communication and education about Medicare MNT and DSMT services.  To provide ADA members with additional tools to increase access to diabetes services, click on the two-page backgrounder and education information below and share this information with physicians and others.  A summary of the Medicare MNT benefit requirements and DSMT requirements are included, as well as examples of how MNT and the DSMT services can be coordinated for qualifying Medicare beneficiaries.
